Holiday Hours

The week of Christmas: The office will be open regular hours the week of December 18th including Saturday, December 23rd. The office will be closed on Monday, December 25. We will reopen on Tuesday, December 26th for sick appointments. Depending on the number of sick children to be seen, we may close early.

The week of New Year's: Between Christmas and New Year’s, we have limited providers in the office. We plan on having regular hours each day. We will have normal Saturday hours on December 30th. The office will be closed on Monday, January 1st and will reopen on Tuesday, January 2nd but could close early.

It’s Time to Get Your Flu Vaccine!

The best defense against the flu is getting your family vaccinated. Viruses do change, as seen in years past, so it’s important to get a flu vaccine annually. This year’s flu vaccine has four strains (quadrivalent) of flu virus, two A (H1N1 and H3N2) and two B (Phuket/Yamagata and Brisbane/Victoria lineage).

Camp and School Forms

We will gladly complete these forms for you. In order to do this, your child should have a well child visit within the past 12 months. Visits for an illness address a specific problem and are not ordinarily comprehensive enough for us to complete these forms. There is no charge for completing these forms, but please allow us 48 hours. Complete as much as the form as possible, including any medical questions.

How can you get the forms to us?

You can mail, fax to 469-916-6738, drop it off at the office, or download by clicking on the appropriate green button below. Just let us know the best way to return it to you.



Most camp or schools provide the parent with the forms as some want very specific information like the child's heart rate after jumping in place 10 times. Check your school or camp's website to see if they provide a specific form. If there is no specific form, most schools and camps will accept the standard UIL form. Click our form button below to download.



Shot Records

If you need a copy of your child's shot record, email your request to This email address is being protected from spambots. You need JavaScript enabled to view it.. Please put your child's name in the subject line and in the body of the message include their date of birth and the preferred method of returning to you. We can mail, fax, email or leave at the front desk for you to pick up. We will notify you in 24-48 hours when you request has been completed.
