School and camp forms filled out while you are in the office for an exam are FREE of charge.
If your form is not completed during your child’s exam, a $10 fee per form will be added to your patient account. These forms can be uploaded below, emailed (forms@pod.pcc.com), or dropped off in office. Standard turn around time for forms is 48-72 business hours.
If you need a SAME DAY form, please drop off in the office at our front desk by 9am, and it will be ready by end of business day. Emailed and/or uploaded forms will NOT be accepted for same day needs. A $20 fee will be added to your patient account for same day forms.
Please ensure all pages within your form are labeled with your child’s name and date of birth!
Please complete the parent’s portion of your form BEFORE sending the document to us.
