As of January 1st, 2021, POD is no longer contracted with Children’s Health. We are using a new service and must now charge our patients an after hours call fee, since we are charged for our after hours RN’s. As always, there is zero charge during regular business hours.

Shot Records

If you need a copy of your child's shot record, email your request to This email address is being protected from spambots. You need JavaScript enabled to view it.. Please put your child's name in the subject line and in the body of the message include their date of birth and the preferred method of returning to you. We can mail, fax, email or leave at the front desk for you to pick up. We will notify you in 24-48 hours when you request has been completed.

 

Medical Records

If you are moving or need a copy of your child's medical records for other reasons, we will gladly provide the first copy to you at no charge. Download the form here. You can fax it to 214-691-0404 or email the form to This email address is being protected from spambots. You need JavaScript enabled to view it.. We will process your request as quickly as possible. It could take up to two weeks.

If you need to transfer records to us from another pediatrician or hospital, you will use the same Authorization for Release of Information form. You can bring it to your next appointment with us and we will gladly send it for you. If you are scheduled for a well child visit, we will need your child's immunization record before any vaccines can be given.
